Job Description
Sunrise Senior Living is seeking compassionate and dedicated Care Managers to join our team. As a Care Manager, you will provide personalized care and support to seniors in our assisted living and reminiscence neighborhoods. This role involves building relationships, assisting with daily activities, and ensuring the well-being of residents.
Responsibilities
- Build relationships with seniors and their families, attending to their care needs.
- Monitor and report changes in residents' conditions to management.
- Participate in the development of Individualized Service Plans and ensure adherence.
- Promote a safe and enriching environment for residents.
- Assist residents with life skills and activities based on their preferences.
Qualifications
- Dedication to serving seniors with excellent customer service skills.
- Positive attitude, flexibility, and teamwork abilities.
- High School diploma/GED and relevant certifications as per state requirements.
- Previous experience in senior care is preferred.
- Strong communication, organizational, and decision-making skills.
