Job Description

We are looking for a compassionate and dedicated Case Manager to join House of Hope Green Bay, Inc. As a Case Manager, you will play a crucial role in providing case management services to clients in our Emergency Shelter and Housing Stability programs. Your responsibilities will include conducting case meetings, developing and monitoring case plans, maintaining accurate records, and ensuring the overall success and well-being of our clients.

Responsibilities

  • Provide case management activities, housing navigation, and case plan oversight.
  • Collaborate with Advocates and the Lead Case Manager to deliver effective client services.
  • Screen clients for program eligibility and compliance with relevant criteria.
  • Coordinate with local agencies and community resources.
  • Ensure a safe and healthy environment in the emergency shelter.
  • Participate in crisis intervention as needed.
  • Propose program enhancements based on client and community feedback.
  • Attend trainings, community meetings, and public speaking engagements.

Requirements

  • Valid Wisconsin driver's license and successful completion of a background check.
  • Degree or relevant work experience in social work, psychology, or related field.
  • Ability to work effectively with diverse populations and multicultural clientele.
  • Strong organizational and communication skills.
  • Experience in providing direct services to individuals from varied backgrounds.
  • Knowledge of local housing programs and community resources is a plus.
