Responsibilities:
- Perform comprehensive assessments to determine hospice needs
- Develop individualized plans of care with the patient, family, and healthcare team
- Maintain ongoing assessments of patient's physical and psychosocial status

Requirements:
- Graduate of an accredited RN program
- Active RN license in South Carolina or compact multi-state license
- 12 months of recent RN experience
- 1 year of hospice care experience preferred
