INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a passionate Inpatient Case Manager (RN) to join our dedicated team at LifeBridge Health. As an Inpatient Care Manager, you will collaborate with the clinical team and medical providers to ensure seamless discharge planning coordination and intervention for patients with complex health needs.

Responsibilities

• Provide discharge planning coordination in collaboration with the clinical team.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among a patient population with complex health needs.

Requirements

• Basic professional knowledge; Bachelor's degree preferred.
• BSN required from CCNE accredited schools.
