Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. In this full-time role, you will collaborate with the clinical team and medical provider to provide discharge planning coordination and intervention for patients with complex health needs. The primary focus will be on promoting patient wellness, improving care outcomes, and ensuring efficient utilization of health services.
Responsibilities

Requirements
- Bachelor's degree in Nursing (BSN) from CCNE accredited schools.
- Experience in case management or related field is preferred.
