INPATIENT CASE MANAGER (RN)

Job Description

We are looking for an experienced Inpatient Case Manager (RN) to join our team at LifeBridge Health. As the Inpatient Care Manager, you will collaborate with the clinical team and medical providers to coordinate discharge planning and interventions for patients with complex health needs. Your role will focus on promoting patient wellness, improving care outcomes, and ensuring efficient health service utilization.

Responsibilities

• Provide discharge planning coordination and intervention.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment among patients.

Requirements

• BSN required from CCNE accredited schools.
• Basic professional knowledge in a specialized field.
• Experience in care coordination and utilization review preferred.
