Job Description
Join our team at UVM Health Network Home Health & Hospice as a Registered Nurse - Intake & Hospital Liaison. In this critical role, you will be part of our Intake and Referral team, serving as the primary link between referring providers, program leaders, clinicians, and patients and their families.
Responsibilities
- Provide education and information about home health and hospice services to patients and referring providers.
- Coordinate patient discharges, review clinical documentation, and assess patient eligibility for services.
- Write orders for care, offer general information to patients, families, providers, and the community.
- Work collaboratively with a multidisciplinary team to ensure seamless transitions of care.
Requirements
- Associate's degree in nursing and RN license.
- Minimum of two years of RN experience.
Additional Information
- On-call duty is not required for this position.
- External applicants are eligible for a $10,000 bonus.
