Atrium Health is seeking a Registered Nurse (RN) for a Home Health position based in Charlotte, North Carolina. This role involves case management, overseeing post-acute services, providing direct patient care, and coordinating with multi-disciplinary teams to ensure quality outcomes.

Job Summary

• Serve as a case manager for a group of patients in home settings.
• Provide direct patient care and supervise LPNs and HHAs according to the care plan.
• Coordinate care with multi-disciplinary teams and other healthcare providers.
• Ensure timely and qualified patient care documentation.

Essential Functions

• Manage patient caseload, assess patient needs, and develop effective care plans.
• Analyze data to identify issues and develop intervention strategies.
• Prioritize patient health, welfare, and safety.
• Oversee plan of care communication and coordination among involved parties.
• Direct care coordination team and perform treatments as per regulations.
• Evaluate patient progress towards goals.

Requirements

• Graduate from an accredited School of Nursing, BSN preferred.
• BLS certification required.
• Current RN license.
• Valid driver's license.
