Job Description
Atrium Health is looking for a compassionate and dedicated Registered Nurse (RN) for our Home Health team in Charlotte, North Carolina. As a key member of our team, you will provide top-quality care to adult and geriatric patients in a home care setting.
Responsibilities
- Serve as a case manager overseeing post-acute services in patients' homes.
- Provide direct patient care and supervise LPNs and HHAs.
- Coordinate care with multidisciplinary teams and other healthcare providers.
- Ensure timely care, proper documentation, and high-quality outcomes.
Requirements
- Graduate from an accredited School of Nursing, BSN preferred.
- Current RN license and BLS certification.
- Experience in home health or related field is a plus.
- Strong communication and organizational skills.
