SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common challenges faced by patients and families.
• Conduct counseling related to death and dying.
• Participate in discharge planning.

Requirements

• Basic knowledge equivalent to a Bachelor's degree.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
