Job Description

We are seeking a dedicated PRN Social Worker to join our team at LifeBridge Health. The role will involve providing clinical social work services and psychosocial assessments to patients and families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address various challenges including adjustment to institutional living and discharge planning.
• Provide counseling on issues related to aging, death, and dying.

Requirements

• Bachelor's degree and 1-3 years of related experience.
• LBSW/LMSW/LGPC certification.
• Demonstrated knowledge of clinical social work theory and practice.
