Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support for various issues such as adjustment to institutional living and end-of-life care.
• Assist with discharge planning and community resource referrals.
• Collaborate with interdisciplinary teams to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW, LMSW, LGPC).
• Knowledge of social work theory and practice.
