Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and their families. Your responsibilities will include addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common issues faced by patients/residents and their families.
- Assist in counseling related to death and dying.
- Collaborate with the healthcare team for effective patient care.
Requirements
- Bachelor's degree with 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of social work theory and practice.
- Excellent communication and interpersonal skills.
