Job Description

We are currently seeking a compassionate Social Worker to join our team on a PRN basis at LifeBridge Health in Baltimore, MD. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, focusing on various common challenges.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and families.
• Address issues such as adjustment to institutional living, financial concerns, family relationships, and more.
• Provide counseling on topics related to aging, discharge planning, and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
