Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges they may face in a healthcare setting.

Responsibilities

• Offer support and counseling for adjustment to institutional living and aging-related changes
• Assist with financial concerns, family relationships, and discharge planning
• Provide guidance on understanding medical diagnoses and procedures
• Offer counseling on death and dying

Requirements

• Bachelor's degree in Social Work or related field
• 1-3 years of relevant experience
• Licensed as LBSW, LMSW, or LGPC
