SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various emotional and practical challenges they may face. Responsibilities

• Conduct psychosocial assessments for patients and families.
• Offer counseling and support for issues such as adjustment to institutional living, financial concerns, and end-of-life planning.
• Collaborate with healthcare teams to ensure holistic care for patients.
• Participate in discharge planning and provide referrals to community resources.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a clinical setting.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
