Job Description

We are looking for a PRN Social Worker to join the team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges. The position involves offering support in areas such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life care.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients/residents and families.
• Address common issues like adjustment to institutional living, financial concerns, and family relationships.
• Offer counseling related to death and dying, discharge planning, and other related areas.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
