SOCIAL WORKER PRN

Job Description

We are looking for a compassionate Social Worker on a PRN (as needed) basis to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families. Your responsibilities will include addressing various issues such as adjustment to institutional living, financial concerns, family relationships, medical procedures, aging-related changes, discharge planning, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common problems.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare teams to ensure holistic patient care.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Knowledge of social work theory and practice.
• Strong communication and interpersonal skills.
