SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker to join our team on a PRN basis at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various social and emotional challenges.

Responsibilities

• Offer support to individuals and families facing institutional living adjustments.
• Assist in financial concerns, family relationships, and coping with medical diagnoses.
• Provide counseling on aging, discharge planning, and end-of-life issues.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• Licensed certification (LBSW/LMSW/LGPC).
• Strong knowledge of social work theory and practice.
