SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various issues such as adjustment to institutional living, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Address common social and emotional issues faced by patients.
• Assist with discharge planning and end-of-life counseling.
• Collaborate with healthcare professionals to provide comprehensive care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed LBSW, LMSW, or LGPC.
