SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker (PRN) to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges related to institutional living, financial concerns, family dynamics, medical understanding, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments for patients/residents and their families.
• Provide counseling and support to individuals dealing with various issues.
• Collaborate with healthcare professionals to create holistic care plans.
• Assist with discharge planning and transitions.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong understanding of social work theory and practice.
