SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients, residents, and families, addressing various common issues such as adjustment to institutional living, financial concerns, family dynamics, and end-of-life planning.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address issues related to adjustment, financial concerns, family relationships, aging, and end-of-life planning.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
