We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning. Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address common psychosocial problems.
• Assist patients with adjustment to aging and institutional living.
• Offer counseling and support related to medical procedures, treatment acceptance, and end-of-life issues.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
• Understanding of psychosocial theory and practice.
