Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. This position, based in Baltimore, MD at Levindale Hospital, involves providing clinical social work services and psychosocial assessments to patients, residents, and their families. The primary focus is on addressing issues such as financial concerns, family relationships, discharge planning, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments for patients/residents and families.
• Provide counseling and support for individuals facing various challenges.
• Collaborate with medical professionals to ensure comprehensive care.
• Assist with discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
