Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges they may face.

Responsibilities

  • Offer support and counseling to individuals and families dealing with adjustment issues, financial concerns, relationships, and medical procedures.
  • Assist with discharge planning and bereavement counseling.
  • Collaborate with healthcare professionals to ensure comprehensive patient care.

Requirements

  • Bachelor's degree and 1-3 years of relevant experience.
  • LBSW, LMSW, or LGPC certification.
  • Strong knowledge of social work theory and practice.
