Job Description

We are looking for a passionate Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services to patients and families.
• Conduct psychosocial assessments and address various challenges.
• Offer counseling related to medical procedures, discharge planning, and other related issues.

Requirements

• Bachelor's degree or equivalent in social work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
