We are looking for a dedicated SOCIAL WORKER PRN to join our esteemed healthcare team at LifeBridge Health in Baltimore, MD. As a crucial member of our Allied Health division, you will provide vital clinical social work services and psychosocial assessments to patients, ensuring they receive the necessary support during challenging times.

Responsibilities

• Conduct psychosocial assessments for patients and families.
• Address common challenges such as adjustment to institutional living, financial concerns, discharge planning, and counseling on end-of-life issues.
• Collaborate with multidisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
