SOCIAL WORKER PRN

Job Description

We are looking for a motivated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of challenges including adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Provide clinical social work services to patients and families.
• Assess and address common psychosocial issues.
• Offer counseling on topics such as discharge planning, death and dying, and aging-related changes.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
