Job Description
We are currently looking for a compassionate Social Worker PRN to join our dynamic team at LifeBridge Health. In this role, you will provide clinical social work services and conduct psychosocial assessments for patients/residents and their families. Your responsibilities will include addressing various challenges such as adjustment to institutional living, financial concerns, family dynamics, medical procedures, aging-related changes, discharge planning, and end-of-life counseling.
Requirements
- Hold a Bachelor's degree and possess 1-3 years of relevant experience.
- Hold a license such as LBSW, LMSW, or LGPC.
- Demonstrate a solid understanding of specialized field theory and practice.
