Job Description

We are looking for a passionate and dedicated Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a range of emotional and practical challenges. This role involves offering support in various areas such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support to patients and families.
• Assist with discharge planning.
• Collaborate with healthcare professionals to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong communication and interpersonal skills.
