Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. This position involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common problems faced by patients and families.
- Assist with discharge planning and counseling related to end-of-life issues.
Requirements
- Bachelor's degree and 1-3 years of experience in social work.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of social work theory and practice.
