SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As part of our Allied Health team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common concerns related to patient and family well-being.
• Conduct discharge planning and counseling for end-of-life issues.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
• Strong understanding of social work theory and practice.
