Job Description
We are currently looking for a PRN Social Worker to join our dedicated team at LifeBridge Health. As a PRN Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Address issues related to adjustment, financial concerns, and aging.
- Provide counseling on understanding medical diagnoses and treatment options.
- Assist with discharge planning and end-of-life counseling.
Requirements
- Bachelor's degree and relevant professional certifications (LBSW/LMSW/LGPC).
- 1-3 years of experience in a social work role.
- Strong knowledge of social work theory and practice.
- Compassionate and empathetic approach to patient care.
