Long Term Services & Support Community Advisor

Job Description

We are looking for a dedicated Long Term Services & Support Community Advisor to join Superior HealthPlan. In this role, you will play a crucial part in assisting our members in accessing employment opportunities and resources. Responsibilities

• Work closely with LTSS providers and account management to ensure adequate staffing levels.
• Organize virtual and in-person job fairs to facilitate recruitment processes.
• Build and maintain partnerships with local educational institutions, workforce commissions, and community organizations.
• Offer support to members in accessing employment assistance programs, referrals, and self-directed services.
• Develop and expand alternative payment models with LTSS providers to recognize quality work.

Requirements

• Bachelor's degree in Human Resources, Health Care, Sociology, or related field.
• 3+ years of experience in employment specialist roles, workforce development, or related areas.
• Community Health Worker Certification preferred.
• Willingness to travel regionally up to 50%.

Pay Range

$25.44 - $45.76 per hour
