Job Description

The Registered Nurse - Maternal Child Health (RN-MCH) at Access Community Health Centers, Inc. works collaboratively as an active member of the interprofessional team to provide therapeutic patient and family-centered care for prenatal, postpartum, and newborn patients up to 1 year after delivery. The RN-MCH systematically and continuously collects and assesses data related to patient health status to develop, execute, and evaluate the plan of care. They utilize evidence to drive clinical decisions and practice related to individual and population health, while establishing goals and strategies for meeting care coordination needs and providing outreach services across settings with the patient, family, and/or other care providers. Additionally, the RN-MCH delivers medical and disease education, actively manages a patient's immediate and ongoing needs with attention to acute and preventative intervention, and considers factors related to safety, effectiveness, and fiscal accountability in planning and delivering care. They also delegate and supervise care in accordance with the WI Nurse Practice Act and ACHC Delegation Protocols, participate in continuous quality improvement initiatives, and assume responsibility for their professional development.

Responsibilities

  • Provide therapeutic patient and family-centered care for prenatal, postpartum, and newborn patients up to 1 year after delivery.
  • Collect and assess data related to patient health status to develop, execute, and evaluate the plan of care.
  • Utilize evidence to drive clinical decisions and practice related to individual and population health.
  • Establish goals and strategies for meeting care coordination needs and providing outreach services across settings with the patient, family, and/or other care providers.
  • Deliver medical and disease education, actively manage a patient's immediate and ongoing needs with attention to acute and preventative intervention, and consider factors related to safety, effectiveness, and fiscal accountability in planning and delivering care.
  • Delegate and supervise care in accordance with the WI Nurse Practice Act and ACHC Delegation Protocols.
  • Participate in continuous quality improvement initiatives and assume responsibility for professional development.

Requirements

  • Graduation from an accredited registered nursing program.
  • Current Registered Nurse licensure in the state of WI.
  • Current CPR certification and maintenance of certification.
  • Current Certification as a Lactation Counselor (CLC), or similar training required within one year of hire.
  • Strong interest and experience in perinatal care and maternal-child health.
  • A minimum of 2 years of experience working with maternal and infant populations.
