Job Description

Genesis Health, a leading healthcare services provider, is seeking a dedicated Licensed Practical Nurse MDS Coordinator to join our team. The LPN MDS Coordinator plays a crucial role in the Resident Assessment Process by completing essential items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure appropriate patient care and nursing center reimbursement.

Position Highlights

• Lead the interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for MDS completion, encoding, and electronic transmission.
• Participate in developing personalized resident Care Plans, addressing resident needs and projected outcomes in line with Federal and State regulations.
• Provide training on clinical content and electronic MDS completion to new team members.

Requirements

• Active LPN license in good standing.
• Prior experience in a hospital, long-term care facility, or similar healthcare environment.
• Familiarity with general, rehabilitative, and restorative nursing practices and regulations.
• Proficiency in computer skills, including windows applications.
• AccuMed experience is a plus.
• Previous MDS Coordinator experience preferred.
• Current BLS/CPR certification required.
