Job Description

Pinnacle Services is seeking dedicated individuals to join our Hennepin County Case Management team as Case Managers. If you have a passion for helping individuals with developmental disabilities achieve their full potential, this is the perfect role for you. As a Case Manager, you will work closely with persons served on waivers, offering resources, monitoring service delivery, and facilitating service plan development.

Responsibilities

• Assess areas where assistance is needed for persons served.
• Coordinate resources and monitor service delivery.
• Develop service plans and maintain client-related documentation.
• Attend meetings with interdisciplinary teams.

Requirements

- Graduation from an accredited four-year college with a major in social work or related fields.
- Valid driver's license and reliable transportation.
- Clear background checks.
- Experience in education or treatment of individuals with developmental disabilities is a plus.
- LSW or BOSW licensure preferred.
