Job Description

We are looking for a dedicated CSC Case Manager to join our team at PermiaCare. The CSC-FEP program provides vital support to individuals experiencing their first onset of a psychotic disorder. As a Case Manager, you will play a crucial role in offering psychoeducation, psychosocial rehabilitation, and case management services to program enrollees. Working closely with a multidisciplinary team, you will help individuals in setting and achieving recovery goals, coordinating services, and providing outreach and education as necessary.

Responsibilities

• Provide direct services and support in alignment with recovery plans and TRR assessments.
• Collaborate with individuals and families to facilitate progress towards recovery goals.
• Ensure the timely delivery of required services and maintain accurate documentation.
• Facilitate education and outreach efforts on First Episode Psychosis.

Requirements

• Bachelor's degree in psychology, social work, or related field.
• 1 year of experience in social services preferred.
• Texas Driver's license and the ability to pass background checks.
• Ability to work independently and as part of a team.
