Job Description
LifeBridge Health is currently seeking a dedicated Complex Care Social Worker - LMSW to join our team at Sinai Hospital in Baltimore, MD. This is a full-time position with a day shift schedule. As a Complex Care Social Worker, you will play a crucial role in facilitating discharge planning for difficult-to-place, high-cost, and escalated patients to ensure timely and effective discharges within the set standards.
Responsibilities
- Utilize a multi-step escalation protocol to identify patients with barriers to discharge.
- Work with the interdisciplinary team to manage escalated patients according to discharge planning standards.
- Monitor difficult-to-place and high-cost patients for early intervention opportunities.
- Ensure timely and cost-effective discharges in line with utilization review guidelines.
Requirements
- Master's degree with seasoned professional knowledge.
- Licensed Master Social Worker (LMSW) certification.
