Genesis HealthCare is seeking a dedicated Minimal Data Set Coordinator - LPN to join our team. As an LPN MDS Coordinator, you will play a crucial role in the Resident Assessment Process by completing key items on the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and reimbursement.

Position Highlights

• Lead the interdisciplinary RAI process ensuring compliance with guidelines.
• Develop individualized resident Care Plans in accordance with regulations.
• Train team members on clinical content and electronic MDS completion.

Requirements

• Active LPN license.
• Prior experience in healthcare settings.
• Familiarity with nursing practices, regulations, and MDS completion.
• Computer proficiency, AccuMed experience preferred.
