Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Assist patients/residents and families with various challenges and issues.
- Collaborate with healthcare professionals to develop appropriate care plans.
- Offer counseling and support to individuals and families during difficult times.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
- Excellent communication and interpersonal skills.
