Job Description
We are seeking a compassionate and qualified Social Worker to join LifeBridge Health. As a Social Worker at Levindale Hospital in Baltimore, MD, you will provide clinical social work services and psychosocial assessments to patients, focusing on various common problems such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and their families.
- Assist patients with discharge planning and address issues related to aging.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- LCSW-C certification is preferred.
- LGSW certification.
