Job Description
We are seeking a compassionate Social Worker to join LifeBridge Health's team at Levindale Hospital in Baltimore, MD. As a vital member of the Allied Health department, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment issues, financial concerns, family relationships, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide appropriate support to patients and families.
- Assist in discharge planning and end-of-life counseling.
- Collaborate with multidisciplinary teams to ensure comprehensive care.
- Advocate for patient needs and rights.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- Licensed Certified Social Worker (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW).
- Excellent communication and interpersonal skills.
