Job Description
We are currently looking for a dedicated Social Worker to join our passionate team at LifeBridge Health. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients, families, and residents, addressing a range of common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to medical procedures and end-of-life care.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist individuals and families in navigating medical diagnoses and treatments.
- Develop discharge plans and provide support for end-of-life care.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
- Experience in a healthcare setting is a plus.
