Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common challenges they may face. This full-time position offers the opportunity to make a meaningful impact in the lives of individuals in need of support.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients/residents and families.
- Address common challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
- Offer counseling related to death and dying, and empower individuals facing difficulties.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred, LGSW certification a plus.
- Strong communication and interpersonal skills.
