SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, addressing common issues related to institutional living, financial concerns, family relationships, medical understanding, aging, discharge planning, and end-of-life counseling.

Our ideal candidate should hold a Bachelor's in Social Work (BSW), with a preference for a Master's degree. An LCSW-C certification is preferred, and LGSW certification is required.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address common issues related to patient and family well-being.
• Facilitate adjustment to institutional living and aging.
• Offer discharge planning and counseling on death and dying.

Requirements

• Bachelor's in Social Work (BSW) required.
• Master's degree in Social Work preferred.
• LCSW-C certification preferred.
• LGSW certification required.
