Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit Physical Therapy assessments, providing direct patient care and evaluating medical conditions for rehabilitation potential.
• Proficiency in EMR documentation (experience in Hello Note preferred).
• CPR Certification.
