Responsibilities

• Conduct home visit Physical Therapy assessments and provide direct patient care.
• Evaluate medical conditions, functional capabilities, and rehabilitation potential.
• Proficient in EMR documentation (experience with Hello Note is beneficial).
• Maintain CPR Certification.

Requirements

• Graduated from an accredited Physical Therapy (PT) program.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Flexible scheduling options available.
