Physical Therapy Specialist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments to provide direct patient care, assess medical conditions, and evaluate potential for rehabilitation.
• Proficient in EMR documentation (experience with Hello Note preferred but not required).
• CPR Certification.
