Physical Therapist Requirements:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Hold an unencumbered Physical Therapy (PT) license for New York.
• Conduct home visit assessments, provide direct patient care, and assess medical conditions and functional capabilities.
• Proficiency in EMR documentation, CPR Certification.
